Request for Amendment of VHA Health Records

An individual has the right to request an amendment to any information or records retrieved by the individual’s name or other individually-identifiable information contained in a VA system of records. The right to seek an amendment of this information or records is a personal right of the individual to whom the information pertains.

Rights of Access

Rights of Access Definition: “An individual’s right to have access to (e.g. look at, view) or obtain a copy of records pertaining to the individual that contain individually-identifiable information.”
